My (prospective) employer has asked me to fill in a Personal Information Submission Form (OPG) for a security screening application they will be submitting. What should I do?

If you have received an email from eOPG (electronic application form), you can fill in an OPG through this eOPG web service. To be able to use eOPG you and your partner (if applicable), should have a DigiD token with 2-factor authentication by SMS.

If you did not receive an email or if you do not have DigiD, then you can download the OPG forms applicable to your (prospective) position involving confidentiality as pdf documents from www.aivd.nl.
