In order for the UVO to carry out a security screening, the person under consideration for or in a position involving confidentiality has to submit personal information. This can be done digitally or on paper. The employer will let the person know how the information should be submitted.

Digitally

If the employer has submitted the security screening through the digital portal (e-AV), the person under consideration for or in a position involving confidentiality automatically receives a link to the online application service for filing personal information in relation to requests for security screenings (elektronische Opgave Persoonlijke Gegevens, eOPG). The person under consideration for or in a position involving confidentiality needs a DigiD to log in. If they do not yet have a DigiD, the application needs to be done on paper. You can find more information under ‘application security screening’. It will take a maximum of five days before they receive an activation code. If they cannot or do not want to apply for a DigiD, they have to contact the employer. The application then has to be done on paper.

Paper

If the application is done on paper, the Statement Personal Details (Opgave Persoonlijke Gegevens, OPG) must be filled in via Adobe Reader and printed. You can find the forms for the different levels of screenings below. There is a different form for each security clearance level. When it is unclear which security clearance level is required, get in contact with the employer.

Important: always use the latest version of the forms
